NC State Division of Environmental Health and Public Safety 

  • Emergency Preparedness: Includes Emergency Management, Fire and Life Safety, Insurance and Risk Management, IT Support/Strategic Initiatives, and WolfAlert.
  • Environmental Health and Safety: Explore safety policies and plans for campus-wide operations. For questions about your area’s business continuity (PackReady) plans, please contact your Senior Management Team (SMT) representative.  
  • Risk Assessment: Discover how to report and share concerning behavior, explore available training sessions, and see upcoming campus events. 
  • University Police: From any campus phone, dial 33000 to reach University Police. From outside campus lines, dial 919.513.3000 or 911 for police assistance.

Report an Accident 

In the event of a serious but non-life-threatening condition, such as a laceration, fracture, fever or vomiting, advise employees, students or visitors to visit a local urgent care facility. For life-threatening medical emergencies, call 911.

Reporting Procedures

Visitor or student (non-student worker) incidents:

  1. The supervisor or other university staff member should complete the Student/Visitor Incident Report Form on the Insurance and Risk Management forms website. Do not use this form for employee incidents.
  2. Please follow the instructions provided on the form and send the completed copy to delta-nscale-hr@ncsu.edu

Student workers and NC State University employees:

  1. If this is the first incident, fill out the Supervisor’s First Report of Incident. 
  2. Return the form to the listed parties at the top of the form and inform HR at delta-ncscale-hr@ncsu.edu.
  3. Review other reporting requirements outlined at the Environmental Health and Safety Accident Reporting website
