NC State Division of Environmental Health and Public Safety
- Emergency Preparedness: Includes Emergency Management, Fire and Life Safety, Insurance and Risk Management, IT Support/Strategic Initiatives, and WolfAlert.

- University Police: From any campus phone, dial 33000 to reach University Police. From outside campus lines, dial 919.513.3000 or 911 for police assistance.
Report an Accident
In the event of a serious but non-life-threatening condition, such as a laceration, fracture, fever or vomiting, advise employees, students or visitors to visit a local urgent care facility. For life-threatening medical emergencies, call 911.
Reporting Procedures
Visitor or student (non-student worker) incidents:
- The supervisor or other university staff member should complete the Student/Visitor Incident Report Form on the Insurance and Risk Management forms website. Do not use this form for employee incidents.
- Please follow the instructions provided on the form and send the completed copy to delta-nscale-hr@ncsu.edu.
Student workers and NC State University employees:
- If this is the first incident, fill out the Supervisor’s First Report of Incident.
- Return the form to the listed parties at the top of the form and inform HR at delta-ncscale-hr@ncsu.edu.
